I OUR SERVICES

Our Services

As an Integrated Service Delivery Model (ISDM), Yancey School Community Center hosts a variety of services, from our community food bank to housing several health & service organizations.

  • An ISDM is a “team-based, client-focused approach to delivering health and social services,” (Source). Services become a single, seamless system as clients and their network of health and social providers closely collaborate to achieve the best care possible.
